Please make sure to bring the following items to your appointment:
FOR EVERYONE:
1) Bring your driver’s license.
2) Drink lots of water for the urine specimen test (checks for sugar, blood, and protein).
3) Bring this completed medical questionnaire to your appointment.
4) Bring $110 payment in the form of cash, check, or credit card.
IF YOU HAVE VISION OR HEARING PROBLEMS, please bring your eyeglasses, contact lenses, or hearing aids.
IF YOU HAVE DIABETES, please bring:
1) The most recent lab results from your Hemoglobin A1C (HgAIC).
2) Your blood sugar logs for the last three months.
IF YOU HAVE ANY HEART RELATED ISSUES, please bring a letter from your cardiologist that outlines your medical history & current medications, and indicates that you are safe to work.
